Characteristics of Lower Extremity Ischemia and its Causes

Ischemia of lower limbs affects arteries and causes their narrowing or even occlusion. There is insufficient supply of oxygen and nutrients to the muscles and tissues, resulting in a variety of trophic changes, which may lead to amputation of a limb.

The most common cause of narrowing or occlusion is atherosclerosis (hardening of the arteries), thrombus (blood clot), Buerger’s disease or vasoconstriction of arteries.

The first stage of lower limb ischemia does not have any symptoms experienced by the patient. Vessels are only slightly narrowed and can sufficiently supply limbs with blood. The affected react negatively to fluctuating temperatures, and will often complain that their feet feel cold when the weather is cold.

The second stage of the disease is symptomized by pain. This stage is characterised by intermittent claudication; this is when the affected individual has to take a break from walking due to pain and may resume once the pain dissipates after a short while. Trophic changes may also appear.

Permanent pain in the limb means that the disease is in its third stage.

The last, fourth stage, is characterised by insufficient blood perfusion; there are muscle twitches and spasms. The affected is in danger of tissue necrosis, which first appears on peripheral parts of the limb.

The main risk factors in developing the disease are:

  • smoking,
  • lack of exercise,
  • advanced age,
  • psychological stress,
  • obesity,
  • high level of cholesterol,
  • high blood pressure,
  • diabetes and others.

This disease requires specialised vascular examination and timely start of treatment based on the stage and location of the disease. The main goal is to improve arterial blood circulation and to avoid amputation.

Adjustments to the lifestyle and elimination of risk factors like smoking, stress, weight reduction and managing diabetes are important.
